Membership Fee
To complete your Executive Assessment request, you need to pay the annual Iasis Safeguard Plus intake fee.
Please use the Iasis Shoppe to pay the non-refundable review fee of $500. You will need your Visa or MasterCard to use the Iasis Shoppe shopping cart.
Once the fee is paid, you will be contacted by the physician's office to arrange your appointment.
The fee at the doctor's office for your two hour appointment is C$2000 and is payable by credit card, corporate cheque or cash.
